Company Contact Information
Pursuant to La. R.S. 22:41.2 and LAC 37:XIII.Chapter 175 (Regulation 117), all risk bearing entities must annually provide notice to the commissioner of certain contact information and must make updates to that information no more than 30 days after a change. You may find a list of “risk bearing entities” in La. R.S. 22:48.
Types of contacts. The following contact types are mandatory for all risk bearing entities:
- Consumer Complaint Contact
- Regulatory Compliance Contact
- Annual Statement Contact
- Premium Tax Contact
- Cybersecurity Contact
- Catastrophe/Disaster Coordination Contact
- Market Conduct Contact
A company may choose to have a “Primary Contact” to satisfy one or more of the requirements rather than naming separate individuals for each position. A company may also have multiples of any contact type.
How to submit annual notice and changes to information. A risk bearing entity may use the Licensee Contacts Module or the Uniform Certificate of Authority Corporate Amendment Form 14 to notify the LDI of changes to the contact information but the annual notice will only be available through the Licensee Contacts Module of the Industry Access System. The annual notice must be submitted no later than March 1 of each year.
Using Industry Access. Any user of the Industry Access System may request access to the Licensee Contact Module for a company and a user with access to the module for multiple companies may copy the information from one company to another with relative ease and without duplicative entries. The Industry Access Account Administrator for the company can determine which users, if any, currently have access to the module.
