IMPORTANT NOTICE: The Annual Premium Tax Statement (Form 1061) and the Annual Municipal Premium & Tax Report (Form 1076) must be filed online through the Industry Access Portal by all companies required to file premium tax returns. (You can no longer download and mail these forms). Proper permissions must be set up for these forms prior to initial access. It is not advisable to wait until shortly before the tax filing deadline since the initial online setup process involves several steps.
Companies claiming the investment tax credit must also complete 2023 Form 1068C - Investment Tax Credit Report of Qualifying Assets. Companies claiming the military tax credit must complete 2023 Form 1425 Military Tax Credit Report Form. These forms must be completed and uploaded when filing Form 1061 to the Louisiana Department of Insurance along with the supporting documents. The Louisiana Department of Insurance also accepts online tax returns filed with software by third-party vendors such as Tri-Tech Software’s Premium Pro tax program.
